1. Demonstrates proficiency and continued growth in the knowledge of transplantation. Demonstrates ability to monitor immunosuppressive therapy under the supervision of a qualified physician.
2. Follows patients waiting for organ transplant on an outpatient basis, manages
3. TRANSTANT waitlist to ensure all candidates are current and up to date on waitlist.• Follows patients to ensure all necessary studies, labs and procedures are current and waitlist patient are ready for transplant, manages refills, completes prior authorizations.
4. Clinically evaluates patients under the guidance of the transplant surgeons/physicians, manages incoming call from patients regarding symptomology and provides direction as needed with appropriate transplant physician guidance.
5. Reviews medications and reinforces pre- operative teaching.
6. Assists the patient and family to cope with the stressors of awaiting transplantation.
7. Provides clinic support to MDs when patients come to clinic.
8. Assist with identification of potential living donor candidates and refers living donors to living donor coordinator.
